Please keep your child's Emergency Card information up-to-date (contact phone numbers, allergies, name of doctor, etc.). You can complete a paper copy provided by the school or update the information online through the Parent Portal.
If you need to drop off medication for your child to have in the clinic, this can be done before school starts.

Examples of Routine Health-Related Services
Physical Health Services:

Injury evaluation and first aid care

State mandated health screenings (vision, hearing, spinal, acanthosis)

Assessment and care for illness or physical symptoms while at school

Heat illness prevention and injury support for student athletes

Hygiene support
Mental Health Services:

Short-term individual counseling

Comprehensive student-success needs assessment (academic related behavior needs like study skills and school involvement, not for mental health diagnostic purposes)

Social-emotional skill-building or group counseling

Support during crisis, including assessment following a reported concern of student self-harm or harm to others

Referral to external providers, if needed (with additional consent)
What this Consent Does Not Include:

Formal medical or psychological diagnosis

Prescription or over-the-counter medication administration without proper authorization

Ongoing therapy or treatment requiring parental notification under law
These activities would require separate written consent and parental involvement, unless in emergency or legally exempt circumstances.
If you do not consent to school health-related services, or do not submit this form, school staff will not be able to provide any physical or mental care for your child - except in cases of a life-threatening emergency.
